OVERVIEW OF THE HEALTHCARE PROGRAM
The COVID-19 pandemic has illustrated just how important our healthcare system is. This pandemic has stressed our healthcare organizations, and every provider has responded to the challenge.

To help lessen the impact of COVID-19, Montgomery County is providing grants to help healthcare organizations (i.e.: hospitals, medical centers, health clinic, and other health service providers). To qualify, your organization must provide public health services to individuals and communities within Montgomery County, Ohio directly impacted by the COVID-19 pandemic.

Grants will be awarded to eligible healthcare organizations. Grant funds can be used for mortgage/rent, utilities, costs related to immunizations, coronavirus testing and contact tracing, PPE supplies, technology, and other public health expenses. These funds can be used for expenses, either already incurred or anticipated, between March 1 and Dec. 30, 2020.

ELIGIBILITY CRITERIA
To be eligible for CARES Act funds, you must be able to prove that the pandemic has had a negative impact on your organization. Other requirements include:

Proposed program services must benefit individuals living in Montgomery County, Ohio
Your organization must be physically located in Montgomery County, Ohio
Your organization must be able to meet program technical requirements, including the ability to provide financial records to support the request (including the ability to reconcile and report the use of federal funds, as well as be able to report the number of clients served and other demographic information
YOU WILL NEED THE FOLLOWING DOCUMENTS TO APPLY
Copy of your current Ohio Secretary of State certificate of good standing
Copy of most recent (2019 and 2018) organization tax return
Listing of current board members
Completed and signed W-9
Copy of Form W-3 Transmittal of Wage and Tax Statements (if applicable)
Copy of Form 1096 Annual Summary and Transmittal of U.S. Information Returns (if applicable)
Proof of actual operating expenditures from January through March, 2020
Projected budget expenditures for operations from June 1 through Dec. 30, 2020
